I acknowledge that running in the Sam Williams Leadership Foundation 5K race is a physical activity. I certify that I am medically able and properly trained. I assume all risks associated with participating in this event including, but not limited to: falls, contact with other participants, the effects of the weather and the conditions of the trail, all such risks being known and appreciated by me. Having read this waiver and knowing these facts and as valuable consideration for your accepting my entry to participate in the Sam Williams Leadership Foundation 5K (herein after the “Event”) to be held on March 22nd, 2025 at Wootton High School, Rockville, Maryland, I, intending to be legally bound, do hereby for myself, my heirs, my executors, and administrators agree as following provisions:

1. I do hereby waive and forever release all persons, entities and agencies involved with promoting and holding the Event, including but not limited to Sam Williams Leadership Foundation 5K, Wootton High School, Wootton Booster Club, all sponsors, volunteers and vendors of the event, their agents, successors, representatives and assignees from any claims for damages and liabilities of any kind arising out of my participation in the Event against whether damages arise of the negligence of an entity or agency involved with promoting and holding the event or the negligence of the participant named in this waiver whose signature is affixed below.

2. I do hereby assume the risk of bodily injuries and personal injuries up to and including death, resulting from participation in the Event and waive all entities and agencies involved with promoting and holding the Event, including but not limited to Sam Williams Leadership Foundation 5K, Wootton High School, Wootton Booster Club, all sponsors, volunteers and vendors of the event, their agents, successors, representatives and assignees from liability for said bodily injuries and personal injuries up to and including death. I also waive the aforementioned entities and agencies liability for any and all loss or destruction of my property, including loss of use thereof and any other indirect or consequential damages, resulting directly or indirectly, wholly or in part, from my participation in the Event and while traveling to and from the Event.

3. I hereby agree, for myself and/or for a minor under the age of eighteen for whom I am signing, to indemnify, defend, and hold the entities named above harmless from and against any and all claims, liabilities, losses and damages, costs, expenses (including attorney’s fees) judgments and penalties arising out of any of my, and or said minors, acts or omissions to act.

4. I understand that Sam Williams Leadership Foundation 5K reserves the right to use any and all participant’s photos with regard to promotional and/or advertising materials including, but not limited to, postings pictures on websites and social media platforms.

5. I understand that all entry fees are non-refundable.

I agree to all rules, conditions, and restrictions listed above for participants, I am registering for the Sam Williams Leadership Foundation’s 5K run/walk.
